Forms

There are several administrative documents required to participate in Sea Cadet trainings. The main form, NSCADM is used to record pertinent personal Cadet information as well as standard releases and privacy statement. You can access the full NSCADM packet (11 pages) or you can download the forms separately:

• Report of Medical History: NSCADM 001 (Pages 3 and 4 Only)

• Report of Medical Exam: NSCADM 001 (Pages 5 and 6 Only) Doctor signature required

• Medical History Supplemental: NSCADM 001 Pages (7 and 8 Only)

• Request for Accommodation: NSCADM 001 (Pages 9 and 10 Only)

• Parental Support Agreement

Uniform

The UNSNSCC uniform manual includes details on:
